How to Revalidate your Medicaid Enrollment

Once notified of the need to revalidate, follow the steps below:

  • Complete the MAS Transportation Provider Revalidation Application to request your MAS Provider Revalidation Letter.
    If approved, supplied revalidation letter will be valid for 90 days
  • Upon receipt of the MAS Provider Revalidation Letter, promptly submit it to the New York Medicaid Program with your revalidation application.
  • Providers who fail to submit a complete revalidation application, including the MAS Revalidation Support Letter, within the allotted time may be subject to termination from the Medicaid Program and the MAS NYS Medicaid Transportation Network.
